The project for CDL Candles is still in progress. We started out with creating a traditional brand identity by designing a logo, from which we could design a business card, an order form and a brochure. In the future, an e-commerce enabled web site will be added to sell their products online.
The jar candle order form for CDL Candles needed a great deal of flexibility as the candles are completely customizable. The customer has the option to order each candle in their choice of size, color and scent.

Cambridge
Partners, Inc.- Web

Cambridge Partners is a property management group that owns six apartment communities in the Lake Orion, Michigan area. They needed a new application form that would be up to par with their new web site and that could be completed online and then printed and either faxed or brought into the office. We designed the new form in Adobe InDesign, then printed it to an Acrobat file. Once in Acrobat 7.0, we put in the fields to make it "electronic", allowing the user to fill the form out on their computer, then print/save the form. This version is made available from the website. A purely print/press ready file was also made available to Cambridge Partners for use in the office.

General Motors
This is an example of a Microsoft Excel form that I created while I worked at General Motors. The form was used to track the weekly activities for each of our team members, allowing for high-level and detailed sections, and also for visual charting. Each individual completed one of these templates, and then they were combined into a compiled report for the entire team.

The Office Grapevine™ - Web | Logo
The project for The Office Grapevine was a great experience, as this was a full identity branding package, including a logo, web site design and development, stationery and forms design. I became very personally attached to this project, and was intimately tied to the design and its outcome. This one was definitely a labor of love, and we are very happy with the results.
When creating the stationery package, an additional logo for the web site address was created to place in the footer, as well as an image for the company's tagline. We created a letterhead, envelope and address label template for Microsoft Word, as well as a press ready address label for printing should the client desire to have labels printed.
We continue to provide web site maintenance services for The Office Grapevine.
Lake Orion Family Dentistry - Case Study | Web
Creating the website for Lake Orion Family Dentistry was both one of the biggest challenges I have faced doing a freelance website, while also at the same time one of the most fun. It is a large site as far as small business sites go, having a large amount of information simply for the purpose of providing the patient with a value add.
There are several forms and informational pdfs available on the website. To the right is one example of a consent form that shows the format that is consistent across all of the online pdf forms and documents.
